1. Registered user can book only his/her own appointment.
2. The patient would be given two complimentary OPD consultations within two weeks of your paid consultation.
3. Beyond two weeks of paid OPD consultation, Kindly make a fresh appointment and pay at the reception counter of National Hospital.
4. First/Paid appointment can not be re-scheduled.
PATIENT RIGHTS:
Right to be fully informed of his/her condition in language that can be understood by the patient.
Right to get respect for any special preferences, spiritual and cultural needs.
Right to be respected for personal dignity and privacy during examination, procedures and treatment, and to be free from all forms of abuse, neglect and harassment.
Right to participate in decision making.
Right to privacy and confidentiality.
Right to refuse treatment.
Right to give informed consent before anesthesia, blood and blood product transfusion and carrying out any invasive/high risk procedures / treatment.
Right to be informed on how to voice a complaint.
Right to be informed on expected cost of the treatment.
Right to have an access to his/her clinical records as per policy.
Right to receive care in safe and secure environment.
PATIENT RESPONSIBILITIES:
Provide complete and accurate information about his/her health, including present condition, past illnesses, hospitalizations, medication, natural products and vitamins, and any other matters that pertain to his/her health.
To provide complete and accurate information for insurance claims and records.
Following the treatment plan recommended by those responsible for their care.
To ask question when he/she does not understand what the doctor or other member of the healthcare team tells about diagnosis or treatment. He/she should also inform the doctor if he/she anticipates problems in following prescribed treatment or considering alternative therapies.
For their actions if they refuse treatment or do not follow the healthcare team's instructions.
To be on time for doctors' appointment.
Following hospital's rules and regulations:
Comply with the no smoking policy. Be considerate of noise levels.
Being considerate of the rights of other patients and hospital personnel.
Not to damage hospital properties.
Weapons are prohibited on premises.
To take care of personal belongings, valuables/money, credit/debit card, etc.
To see their daily bill updates.
To pay for services billed for in a timely manner as per the hospital policies.
To respect that some other patients medical condition may be more urgent than yours and accepts that your doctor may need to attend them first.
To communicate with the healthcare provider if his/her condition worsens or does not follow the expected course.
To accept the measures taken by the hospital to ensure personal privacy and confidentiality of medical records.
Not to take any medication without the knowledge of doctor and healthcare professionals.
